I.- INFORMATION PRIOR TO THE FORMALIZATION OF THE CONTRACT

In order to formalize this contract, you must follow the following steps:

1.-You must fill out all the information required on the donation forms.

2.- The general conditions must be accepted before sending the form.

Once the entire form has been completed, payment will proceed. When your donation has been made, you will receive an email thanking you for your donation. This email confirms that the request to make a donation has been correctly processed by the entity.

3.- The donation report will be filed in our system. To access it or to correct errors write to: donations@neuraltherapyrf.org . You can also contact us at the telephone number +34 93 727 80 68.

II.- IDENTIFICATION OF THE PARTIES

On the one hand, Neural Therapy Research Foundation with address at Alfons Sala, 75 08202 Sabadell, Barcelona, and CIF G67522656 registered in the Registry of Foundations of the Generalitat of Catalonia. On the other hand, the donor, whose details are included in the attached form, who declares that he is of legal age.

III.- OBJECT OF THE CONTRACT

The purpose of this contract is to make a donation to the Neural Therapy Research Foundation.

IV.- PAYMENT METHOD

When card payment is chosen, you will be redirected to your bank’s payment gateway. Any errors on this page should be reported to your bank.

V.- USER OBLIGATIONS

The user is obliged to:

a.- Make the donation for the selected amount.

b.- Receive the tax certificate corresponding to your donation. In the month of March of the year following your donation, you will receive by e-mail the corresponding certificate to be able to deduct your donation. Only applicable for donors who file their taxes in Spain.

VI.- OBLIGATIONS OF THE NEURAL THERAPY RESEARCH FOUNDATION

Neural Therapy Research Foundation is obliged to send by e-mail the tax certificate of the donation to the email address indicated by the donor.

VII.- CANCELLATION OF THE ORDER

The donor may cancel the donation or reject its amount up to 24 hours after making it by sending an email to donations@neuraltherapyrf.org or calling +34 93 727 80 68.

IX.- JURISDICTION AND APPLICABLE LAW

The relationship between the Neural Therapy Research Foundation and the donor will be governed by current Catalan regulations and any controversy will be submitted to the Courts and Tribunals of the city of Barcelona.
